Donations to PRASAD CDHP

Honor / Memoriam Gifts

Consult with your financial planner, accountant and/or attorney to determine which categories best fit your individual and family situation.

A meaningful way to express your sympathy when a close family member or friend passes is to make a memorial gift to PRASAD CDHP. Also, consider honoring someone living by making a donation to PRASAD CDHP in lieu of buying a gift. Occasions for making a donation in honor of someone include:

2. Birthday
3. Graduation
4. Wedding
5. Anniversary
6. Mother's Day
7. Father's Day
8. Grandparent's Day
9. Christmas
10. Hanukkah
11. Any special occasion for which you would buy a gift
    

To make an "in memory" or "in honor" of tribute, please be sure to let us know the following information at the time you make your donation to PRASAD CDHP:

1. Your special intention for this gift (whether it's "in memory" or "in honor" of someone)
2. The name of the person you are remembering or honoring
3. The reason or occasion for giving the gift
4. The complete name and address of the person you would like PRASAD CDHP to notify of your donation (we do not include the amount of your gift). If remembering someone who is deceased you may wish to indicate a family member or close friend of the deceased for PRASAD CDHP to notify.
5. Please be sure PRASAD CDHP has your complete contact information so we can reach you in the event there are any questions.
